Yeah. I think that is for the audience's benefit.
For it to be a truly accurate take on western anime freaks it would be written in sub-par Japanese with random English words dotted about the place for no reason.

I don't have a doctor, I'm on a list waiting for one. I used to have one but I stopped going to him & he retired so I was lost of the great bureaucracy void.

I could join a private clinic & pay for one but I don't want too, so hence I remain doctor less.
