Always seems to only apply to someone who's anorexic or bulimic. I can't remember hearing it used to reference someone who just eats too much.

Its a question of degree. If you define a large percentage of the American population as having an eating disorder simply because they tuck into too much fast food then the word loses meaning.

Actually I did come across someone talking about fat acceptance in the gay community. I was a bit sickened.

I guess you're right that things like obesity generally get discussed as completely separate concepts but it isn't like obesity doesn't get discussed.

An observation that you are having a bad brain function day isnt an ad hom. An ad hom would be something like you are a gun nut and so cannot be right about this. Well anorexia and bulimia are not simply under-eating either. They are very serious life threatening disorders. I think the equivalent for the fatties would be balooning to 500+ pounds.
