What does a TRUMP presidency look like?

My state cooperated with the exchange, but so far it's only working in two counties. The other 15 only have one insurance company available. The two counties are the only blue ones, Washoe and Clark, where Reno and Vegas are.

So the urban-rural divide might explain more about where ACA is more and less successful.

That's the unfortunate part of for-profit insurance companies.  There is no good and compelling reason not to participate in certain states and certain regions, other than basic fucking greed.
