It is a common anime theme.  Usually done by school girls.  So the photoshopped image is basically saying, she is like a school girl.

It is a common way to introduce a character.  Why does she need to put a toast in her mouth instead of eating it at home?  Because she is late for school.  That means she is tardy and prone to mistakes.  But she is not a total bummer, because she still cares and she tries to reach school before the bell rings. 

Having a toast in her mouth, usually with jam on top of the toast, also creates opportunities for interactions.  She is in such a hurry that she crashes into her crush, resulting in jam all over his uniform etc.  So she will have an excuse to call him etc.

The picture I posted is that of Rei Ayanami, from the anime Neon Genesis Evangelion.  Even non-anime fans should be faintly aware of that title.  It is a cultural icon and received lots of critical acclaim.  It pretended to be a run of the mill mecha show when it started, but gradually turned into a horror, psychological, philosophical and also deeply convoluted mess that failed to resolve itself during its original run.  It is one of the most well-known anime out there, watched by lots of adults. 

The use of the toast in NGE is brilliant.  It actually appeared in episode 26, the last episode, instead of episode 1, which is the usual practice.  Rei Ayanami, if you know her, is a silent and emotionless character who does what she is told.  Episode 26 is when the production team ran out of budget but couldn't resolve the story.  So they decided to create this alternative world where all the characters lived normal lives, and everything happened before episode 26 was sort of a dream.  Rei running with the toast signifies that she is now a normal schoolgirl rather than the deeply disturbed, emotionally stunted mecha pilot when the show started.

Figure heads don't normally execute rivals.

Or the non-figureheads do it and pin it on the figurehead that legitimizes the regime. It's really impossible to tell with North Korea.
