I'm just wondering how this goes for other people (especially for heteros) since I don't know last names of most people I fucked/went out for one or two dates with.

Last name. First name last name.
I don't date.
If I'm speaking to a random girl at a party or whatever I usually don't ask about names, names are boring, why does she care what your name is if you're just some random guy? Names come when you're well into the conversation and it has been established its worthwhile getting to know each other. Or when she asks as its social convention to do so.

I always gave the whole name if we actually made it a date. But I was always very clear I was going for something long term and did not have much interest in hooking up.
I believe that my wife would frown on me going out on a date. If by "frown" we mean "slit my throat in my sleep".
