No it was a sub in play in early 2nd quarter, Niners did the same with Kapernick who scored the first TD.

what happened?  I noticed while waiting in line that hasselbeck was in.

Glover Quin hit him with the blindside safety blitz, which nobody picked up.  At the same time, JJ Watt beat the tackle like a rented mule and so Locker got double hit.  Looks like he either broke his finger or wrist or hurt his shoulder.  It wasn't an especially an awkward landing.
