According to this 2010 chart public+private expenditure in the US would be a "mere" 2,67 more than in Spain. Not 3,2 but in the ballpark. The US just has some frankly ridiculous health costs.

Note that we've cut public health expenditure since 2010 so the difference will possibly be higher now, dunno which data the app is using.
